A Day in the Life of an Investigator at Insights25

That’s where the Prisons and Probation Ombudsman (PPO) step in. We are an independent Arms-Length Body investigating: 

• Complaints from prisoners, young people in detention, individuals under probation supervision, and those held under immigration powers. 
• Deaths of prisoners, young people in detention, residents in approved premises, and detained individuals, regardless of cause. 
• Deaths of recently released prisoners within 14 days of release (excluding homicide). 

Complaints from prisoners and young people in detention are handled by Independent Prisoner Complaint Investigations (IPCI), which is part of the PPO. We also gather insights, identify trends, share lessons learned, and occasionally carry out special investigations outside our usual remit. 

Insights25: A Day in the Life of an Investigator 

last year’s Insights event gave colleagues across the criminal justice system the chance to step into the shoes of a PPO investigator. The response was incredible,  with sessions fully booked. 

The day began with introductions and an icebreaker, followed by videos and presentations that set the scene for our work. Delegates explored the PPO’s role, learned how investigations are conducted, and discovered the complexities of working with prisons, prisoners, stakeholders, and bereaved families. Videos featuring prisoners and staff brought real experiences into the room. 

Hands-On Experience and Learning 

Delegates took part in four interactive workshops on fatal incident investigations and complaints handling, gaining practical insight into the challenges our investigators face. PPO colleagues joined the delegates for a networking lunch to allow delegates to share ideas and learn more about our work. 

We also showcased how lessons from cases shape policy and practice. By analysing data rigorously, we identify trends and systemic issues that inform our priorities. Examples of our impact include: 

• Influencing prison policy nationally and locally. 
• Helping prisoners recover or receive compensation for lost possessions. 
• Improving complaints processes for timely responses. 
• Providing answers and support to bereaved families. 
• Highlighting risks and preventing self-inflicted deaths. 

Delegates also saw how our Learning Analysis and Business Services (LABS) team turns findings into actionable insights and publishes research that drives change.  

The day ended with a lively panel discussion with Deputies and Investigators, reinforcing our commitment to openness.
